City of Sturbridge Requirements

If you are an artist/vendor wishing to rent a booth at the convention:

1) Make sure you fill out and return your registration form with payment/deposit....SOON! Payment is expected two-weeks after receipt of form. http://www.zazaink.com/mtaf09appform.html

2) If you are a tattoo artist/body piercer please contact us so you can get the correct paper work in for your temporary permit. You MUST meet all Sturbridge requirements or they will NOT let you work the convention! No exceptions!

FOR ARTISTS WITH 5 OR MORE YEARS EXPERIENCE:

a) All artists must have proof of current Blood Bourne Pathogens certification
b) CPR/1st Aid if you have it, please show it. If not we are covered by the staffed EMTs.
c) Photo ID
d) Copy of your current license

FOR ARTISTS WITH LESS THAN 5 YRS EXPERIENCE:
a) Current Blood Bourne Pathogens certification
b) CPR/1st Aid Certification
c) Photo ID
d) Copy of your current license
e) Skin Disease Course certification. ***

*** If you have not taken a "skin disease course," it will be offered on Friday, March 20th at 10:00am in the seminar room, free of charge, to accommodate the requirements.

The permit fee is $75 per applicant. Please make check or money order payable to the Town of Sturbridge. Send all applications and fees to Zaza Ink, Attn: Irene, 291 West Boylston St, West Boylston MA 01583.

3) Tattoo artists/piercers are responsible for your own booth floor covering. Please contact Irene if you are traveling from out of state and we'll make arrangements for you.
